What Is Mental Health Navigation?

Mental health navigation is a unique service that helps Summit County residents walk through the complex system of mental health care and gets people connected to the appropriate resource when they experience barriers to access.

Building Hope has Care Team Members available to help you determine the best mental health resources for yourself (including bilingual support in English and Spanish).

Please email info@buildinghopesummit.org asking for mental health Navigation services to get connected to a Care Team member and to obtain the support you need.

Please note that Navigation with a Care Team Member is NOT AN EMERGENCY RESOURCE.

What Can a Building Hope Care Team Member Do For You?

  • Help reduce or eliminate barriers to finding a therapist and accessing mental health care
  • Guide you to the community resources and support groups to help you get the care you need
  • Assist you with understanding insurance benefits and applying for mental health scholarships if needed
  • Educate you about available treatment options and other supports in Summit County

*While the navigation program is meant to feel supportive and our staff are trained extensively in the mental health field, it is not designed to be a substitute for professional mental health counseling or substance use treatment*

What is the difference between Mental Health Navigation and Case Management?

Navigators vs Case Managers

Health navigators and case managers are health care workers who aim to provide individualized assistance to individuals with health concerns.
• Some overlapping functions
• Common goals such as improving the client’s health and well-being, and optimizing the use of available resources.
• Terms are often used interchangeably


Case managers usually have specialized training to provide case management services.
• A client is usually assigned a single case manager who acts as a primary contact and advocate for the client, and coordinates the services from/between different providers and agencies.
• Case management is often used for clients with chronic or complex conditions, such as mental health conditions, substance use issues, or when experiencing homelessness.

What is Case Management?

Case management can include several elements such as:
• Needs assessment/screenings (e.g. SDOH, medical needs, behavioral health conditions)
• Individualized care plan development
• Intensive, long term care coordination
• Personalized placement to out-of-county referrals
• Consistent monitoring and follow-up
• Ability to re-evaluate and change the plan as needed.

Patient navigation seeks to guide patients through the healthcare continuum to facilitate timely access to care and foster self-management and autonomy through education and emotional support.
• Patient navigators have familiarity of services and of available resources, and help patients navigate to and access existing services.
• Patient navigators can make simple referrals, provide local resources, and coordinate basic care.

Is Navigation or Case Management therapy?

No, while these programs are meant to feel supportive and staff are trained extensively in the field, it is not designed to be a substitute for professional mental health counseling or substance use treatment.
